1. Test your webcam and microphone prior to your appointment
2. Identify a quiet and private space in your home for your session
3. Prepare any questions you have in advance and keep them close at hand
4. Complete the appropriate intake form from the list below:
   • Cannabis - NEW Patient Intake Form
   • Cannabis - Recertification Intake Form
   • LDN - Patient Intake Form
   • General Appointments - Patient Intake Form
   • General Appointments - Weight Management Intake Form

Please ensure to complete your form at least TWO hours before your scheduled appointment.

Required Information

• Virginia Drivers license or ID
• Physical Medical Records Documenting the Health Condition(s) for which you are being evaluated; or signing the attestation that the information you are providing is accurate and true. You will find this attestation on the bottom of the Patient Intake Form.
